IN ADDITION TO YOUR REQUEST, YOU MUST ALSO PROVIDE LEGAL DOCUMENTATION TO CLAIM FUNDS
In order to process your request, we must receive a notarized copy of all of the following:
- A death certificate.
- Proof of Executor or Personal Representative appointment
- Chugach Certificate of Entitlement Form (Download: Word, PDF)
These documents must be submitted either:

Please submit the above documentation within two (2) weeks of your
Capital Credits research request. If we do not receive your documentation
within this time, a Chugach representative will attempt to contact you

Then, download, complete, and notarize the Chugach Certificate of Entitlement Form (Word, PDF).
Then submit both forms by mail or in person, along with notarized copies of the death certificate and the Proof of Executor / Personal Representative Appointment.
